SCHOOL OF REVIVAL APPLICATION
We are so excited you are interested in applying for CT: School of Revival 2019! We look forward to talking with you more and answering any questions you may have once your application has been received and processed. We recognize this application may seem thorough, but it is helpful to us as we begin to get to know you. By honestly and completely filling out this application you are helping us identify your long-term vision, as well as strengths and weaknesses in your life that we desire to see God develop for His glory.

TRAINING PROGRESSION:
The training consists of two phases, the Nashville Phase and the Outreach/International phase. The Nashville Phase is designed to equip leaders with Godʼs heart and Kingdom plan of revival for Israel and the nations. This is accomplished through teachings from world-class speakers and through immersion into Kingdom culture and community. Topics that will be taught and imparted are: intimacy with God, identity & destiny, worship & intercession, biblical plan for Israel and the nations, and evangelism. The Outreach Phase further imparts Godʼs heart and purpose for Israel and the nations into the lives of the students as they experience the people and the Land of Israel. The Outreach Phase provides the opportunity to put into action what has been imparted. Students will develop relationships with Jewish and Arab believers, receive on-site teaching at biblical locations, and engage in service to the poor. Israel is <1% reached with the Good News of their Messiah. Students will have the opportunity to share the gospel with Jewish and Arab people throughout The Land who are considered unreached people groups.

APPLICATION INSTRUCTIONS:
Please answer all questions honestly and thoroughly. Use the application form and attach additional files as necessary. This application may take you a few hours to complete and must be completed in one sitting as there is no option to save your answers and return later to finish. Start early and turn the application in as soon as possible.

Before you begin, make sure you have the following:

• Detailed medical information (insurance carrier, policy number, expiration date, contact, current physician if applicable, contact info for physician)

APPLICATION FEE:
A non-refundable $35 application fee is required for your application to be considered valid. If your application is accepted, the $35 will be applied toward your tuition cost.

REFERENCES:
You are required to have 2 references submitted on your behalf in order for your application process to be considered complete. One reference must be from a pastor or ministry leader, and the other from a current or previous employer/supervisor or close friend/peer. References provided by family members will not be accepted.

The required Personal & Pastoral Reference Forms can be located on the "School of Revival" page of our website. As the applicant, you are responsible for providing those agreeing to refer you with the Reference Form, as well as following up with those referring you to ensure that they submit your requested reference directly and promptly to Caleb Company.

Upon receiving both references directly from those referring you, a Caleb Company staff member will contact you within 7 days to set up a FaceTime or Skype interview.

All applications and references must be submitted to Caleb Company no later than March 2, 2019.

REQUIREMENTS AND POLICIES:
Please review requirements and policies below and make note of any questions you may have. One of our staff will be glad to answer your questions when they contact you after your application has been processed.

TOTAL COST: This covers food and housing in Nashville for the five-day training week (M-F), CT: School of Revival-related travel during the Nashville portion, and all supplies and activities during the first phase. Your total cost also includes the International phase, all food, housing, airfare* and travel expenses, visas, and sites.

*The International costs are subject to change based upon flight rates at the time of the program.

ADDITIONAL FINANCES: Bring money for additional food, desserts, coffee, souvenirs, and extra activities as desired, etc.

COMMITMENT: Full-time commitment to CT: School of Revival. Students are committing to be open and accountable to Caleb staff, and to participate in all discussions and discipleship activities relating to the training.

TIME WITH GOD: Students will practice a lifestyle of intimacy with God by cultivating personal time with Him.

REQUIRED ASSIGNMENTS: Students must purchase and read “Your People Shall Be My People” by Don Finto before the first day of the training. A 1-page reflection paper will be due before the first day of the training. There may be other books and assignments required during the course of the training.

INITIATIVE: Students are expected to take initiative during worship, discussions, in personal cleanliness, and in serving others. Students might be asked to share what they have learned, or develop their own teaching about the subject matter.

TEACHINGS: Be present and participate in daily teaching times, taking notes, and actively listening. Due to our short lecture phase, each class is extremely important to the training.

TEAM-BUILDING: Students will participate in all team-building activities, such as low ropes and high ropes courses, working together during our work days, and any other team-building activities scheduled.

OUTREACHES: Students are expected to joyfully participate in all local and international outreach activities, with servant hearts full of love.

PHYSICAL ACTIVITIES: The terrain and pace of the International Phase can be physically intense and involve several miles of walking a day in extreme heat. Students will be expected to participate in all physical activities including weekly group exercises and physical training in preparation for the International Phase. Students are expected to keep up with the team in Israel, as well as participate in all physical activities while in the Land.
